Matchmaking mechanic concept: Add a block function.
How the function works: Can block a player in the post game lobby, preventing him/her from ever being queue'd up into the same team and/or match as you again.
This would work to:

Allow players to avoid people they do not wish to play with in the future.

Can potentially mark hazardous people who are obviously causing problems for others by looking at how many times a player has been blocked, which is much easier and more accurate (in some cases) than the report system.

Would act as an off side punishment for excessively negative players as constant blocks could increase his/her queue time which would mean people would also be affected by him/her less.
How this should be implemented (just a hypothetical if this was put into the game): It should be very easy to access with a big 1-click button.
A large amount of players tend to not use the report system as some just want to move on rather than put time into using the report function when it's more that likely not going to do anything, unless they can get several people to do it, and even then it's a slim chance. Another large portion of the community also believes that the report function just doesn't work period.
So by making it as a simple, 1-click button that's right next to the person name. it makes it more convenient to use and would probably have more people using it as it's guaranteed to make sure they don't have to deal with that person again.
