Has positioning become a forgotten technique?

I just don't understand why my ranged (AD and AP) carries often maneuver themselves right into the enemy team instead of using their range advantage. I'm talking about the ranged carries who only goes in Leeroy Jenkins to "melee" the enemy front line (An acceptable exception would be , but it's still risky to let him lead the charge).
