It feels like they don't do much, like grievous wounds are an afterthought thrown onto items like thornmail and morello to increase their cost. Healing seems to be balanced around grievous wounds being readily availible which if you have none you're screwed but if you get it you still won't be on even playing field and might have built suboptimally.
What if grievous wounds increased in intensity the longer you did damage?

Maybe that's just me, though.

I think the grievous wounds effect could work on a different item, but not Thornmail. It doesn't make much sense and it's definitely not strong enough to warrant using it lol
SpaghettiVase2 votesIt's a bandaid fix that exists only to really screw over supports, nothing more. Champions that heal a shit ton in combat 
are balanced around the fact that GW is always gonna be applied to them so their heals are often so strong that GW doesn't do anything. It's not as a legitimate counter to these two as everyone would make you believe
