Doubt I would react quickly enough to do so.
If your life depended on it, probably. Sometimes that half a frame of getting cc'd is more than enough to get you killed, even if you QSS. Better safe than dead.
I will if I can't afford taking the damage from the ability and I don't want to burn flash. Nothing wrong with it, just don't blow the QSS if there is an easy follow up cc that will likely follow.
