Yes, it applies to all damage you deal, with no AoE penalty. Note that ranged champions only get 8% rather than 15%.
Just like KFCeytron said, it works on all damage you deal, including true damage. Conq will pretty much allow Fiora to trade into Tabi Bramble tanks and beat the shit out of them without losing much health. Same with Camille.
