I tried Azir with Scepter in ARAM

Granted this is ARM, so it isn't a grounds for appropriate balance discussions. But holy shit it makes it much easier for his soldiers to get consecutive hits out.

I don't play Azir much at all and when I do I always end up shooting my soldiers too far out for them to auto attack or my opponent doesn't stay long enough to get AAed twice. But scepter just makes it so much easier to deal that damage. Combine his sustained damage with the burst of thunderlords and anyone squishy just straight up dies.

Sorry to be kicking a dead horse, but I think scepter (and maybe Azir but Riot's already acknowledged that he needs work) needs a cooldown or something. On a sustained damage AP champion it will never let up. On a burst champion you'll at least get some window of escape, but the window is nearly always closed if a DPS AP champion catches you in their combo.

Worse still is that any changes to lessen how much you can apply scepter (and I guess frozen mallet) would pretty much gut the champions that rely on it.

Mordekaiser and Gnar are the first to come to mind (there's Yasuo but to be honest I don't think anyone likes fighting Yasuo with Frozen Mallet).

Though this makes me wonder: If a champion hinges on constantly slowing the enemy to actually fight effectively, can they be considered healthy since they need to have a constant advantage to fight?

Take Gnar with Frozen Mallet. With it he can basically kite for days (and he can with BC yet it's not as painful to fight) and his opponent won't be able to escape without burning flash or a mobility spell. I'm not sure Gnar even needs items to help him kite.

Imagine if there was an on hit item that kept applying the grounded effect. Wouldn't that be absolute cancer to any mobile champion since it cuts out a huge part of their kit and with it their fighting power?

I have nothing against items that can slow, but I have to wonder if balancing someone around needing an item to do their job effectively is considered healthy at all (though I guess balancing them to work without items would make them super oppressive with those items).

At this point I'm just rambling. I probably don't really know enough on this topic to have too much of an opinion honestly.

So TLDR: Is requiring constant slows to do your job unhealthy and if so, how would we fix that?

That's not what it is about. The fact that Azir builds item 3116 is due to the synergy it has with his Kit. Azir is in a horrible spot right now, but can be played well and can win in some matchups, he's pretty fun on ARAM. The point I'm trying to make is that his kit does not revolve around item 3116, rather, it compliments his kit so well that it is almost 100% required. Honestly, there are not many ways to build a mage. item 3003 and item 3027 is bad on him. You only really want to build item 3115, item 3116, and item 3157. You can go other AP items from there.

All in all, scepter and mallet have their problems, but it is not JUST the item, it is the synergy it has with a champs kit.

Yasuo wants more mobility then the person he is dueling, so he gets item 3046 and item 3022.

Gnar wants to keep you in place, so he gets item 3022.

Azir gets item 3116 so he is able to deal as much dmg as possible with one W and Q.

All in all, item 3116 and item 3022 don't need a massive change, they are inherently powerful items. They just need a bit of balance tweaks.
