Fiora vital question.

So I've noticed this a few times, where do you need to be standing to proc a vital? For AA wise, because when i ult or Q, it sometimes doesn't proc, and when I AA it sometimes doesn't proc at all, and its very annoying. Am I doing something wrong? Do you have to be in the direction the vital is and attack from there exactly, not even be slightly off? Thanks guys.
