Team diver champions?

I'm looking for champions whose team fight gameplay goes like this:

1. Jump in, blow all your cooldowns on their carry to kill them.
2. Auto stuff and hope the enemy team attacks you or a tank.

List the top 1-5 champions who do this in order of how well they do it.

Thanks! [slayer-jinx-wink]
