Get conditioning.....don't get hit by anyone using sudden impact after they use their gap closer until the 10 minute mark...….you just countered sudden impact along with half a dirk.

Conditioning actually has a horrible bug on it since release that doesn't give you all the armor you are suppose to get.

You've also probably died due to the damage from Aery, Comet, Scorch, Celerity, Absolute Focus, and Gathering Storm. Should remove all those runes as well.
