How Do I Rank Up?

I honestly dont know what to do. For the past few weeks ive been going from Bronze I, to Silver V, to Silver IV, back down to Bronze I, back up to Silver IV, and now im back in Silver V making my way to Bronze again. I repeatedly get in games where either im having a bad game, or my team doesnt do their role. The support will go full ap for some reason, no sightstone, no vision, no helping adc. Top lane (me), will end up getting camped by enemy jungler, ill ask for help when I know jungler is there, and id get no help. ADC will get grabbed from blitz or will get ganked all the time. Mid never comes out of its lane to help lanes, and for some reason never wards. I dont know what to do! I see Diamond League smurfs starting in bronze and they get out so quick to plat its amazing and annoying. How am I suppose to carry my team? If I gank 24/7, ill lose cs. Especially if Nasus is top lane, then there is no way im able to gank other lanes. Also I main Jayce top, so I dont have a lot of mobility.

I honestly want to know, is there a video or something that can explain what im suppose to do. Some games i even go 20/5/11 as Jayce and still lose! Im getting sick of the game, being in the same ranks with no progression at all. Ive tried tons of other champs, but Jayce is the only one i find fun. I dont want to do a champ like Kayle where i auto attack all game, thats boring as hell.
