I have an idea for all these hard to balance champs

Let's take Azir for example. Why doesn't Riot try buffing his BASE damage little by little, I'm talking like 5 damage per rank and see what it does? Sure it might take a while to get him into a good spot, but it will guarantee that he isn't pushed over the top from 1 patch. Don't touch his ratio's though, it's harder to balance by ratio's cause it scales far better than bases and one buff or nerf to an item can completely break him or trash him.

Just a thought IDK.

I could see how this could work, but if they were making slight adjustments to EVERY SINGLE CHAMPION, monitoring would become difficult. I know not every champion requires adjustments, but any change, no matter how small it is perceived, will have an impact on the game, for good or ill.

Yes this is true. But I think in particular this method would work good on champs like Azir, Kalista, Kassadin etc. Since they are generally hard to balance when their ratio's in specific are touched. I mean look at all the nerfs Kass received after his rework. A good portion were to his ratio's, and he became very underplayed afterwards and tanked in W/R. Riot needs to stop touching ratio's and start focusing more on Base damages. Base damage is all of a champions early game, it can make them better or worse.

Sometimes though +5/-5 dmg may break a champion
