Riot hasn't publicly said what goes into the grading system, so maybe is the only solid answer you can get.

Your rank doesn't matter. It's how you match up vs other people playing your champion (and probably role in dynamic).

0/0/30 would require being on a support to get an S.

And I think it depends on game length. In a 70 minute game, the answer is probably no. In a 20 minute game the answer is almost certainly.

I got two S+'s on Sona in the same day from scores of 4/3/27 and 1/8/26, with pretty good particaption on all buffs, towers, inhibs plus almost 40 wards placed in both matches. So yeah, I'd say there could be anything that goes into it that you have to watch.
