Advice on Building Counter play

I was playing jg (not normally my best position to be in) but it was going pretty well I built to help with their strongest Champ but we made some mistakes on the team side as is common in low skill normals and their & got full life steal/atk speed builds in the late game that we never should have let them get to. Now instead of a 4000+HP Beast of destruction I am hitting Q then W then Dying in less than 5 seconds.
In hindsight I think I should have changed up the Items I had. but the question is what would have worked best? I am thinking something like
