How do people play her without getting carpal tunnel?

The vast majority of her mechanics involve mashing the attack move button.

Attack move prioritizes a target within the champions attack range as they move toward the cursor, prioritizing champions over minions (I think, something changed about that recently to make it easier) but still targetting the closest target to the cursor. On ranged champions, because their attack range is large, you can actually alternate between walking and attack moving backwards (aka the cursor isn't near the enemy champion but is instead always behind your champion) to kite much easier than using the mouse.

Kalista is special because of her passive, which allows for movement in between auto attacks and also makes it so that auto attacks can't be cancelled. What this means is that she can spam her attack move button behind her and she will auto the closest target and in between will jump backwards. All you have to do is mash the attack move button.

When I played her during free week I tried doing that. It was not comfortable.

There is a reason Riot said that they wanted to release another, easier adc, because Kalista missed the mark for a lot of people.

I agree with you, the mechanic is strange, its weird to need to hit a button to auto attack with her (not that you actually have to, but you understand my meaning). I don't like it either, but clearly there are a number of people that don't mind it. It just means Kalista isn't the adc for you. But you asked how players were able to do that with her. Thats how.

By removing all skill involved by building hurricane, shift clicking the ground, and hitting E once in a while. Congrats, now you can pretend you're good at ADC by playing a mistake champion who plays the game for you.
