Getting caught out.

Seriously, if there is a chance you will die, assume that you will. Go the safer paths, not the faster paths. Pay attention to your positioning before, during, and after teamfights. Your mistakes add up in this game, and eventually, youve thrown the game.

That and getting cocky.

I've had so many games where we are winning, so your ADC thinks he can splitpush to the enemy inhib while the rest of us are on baron. Just stay with the team and keep playing safe.
