It's meant as a combo stack for armor mostly. Lifesteal gains percentage of damage done, whereas thornmail's passive occurs before armor reduction. Also, you shouldn't take it just because of an ADC. It's more effective against teams with multiple AA reliant champs. Say they have Vayne, Tryndamere, Yi, Sona, and Yasuo. Other than the Sona that team is basically thorn damage material. It only returns a third of the damage but that means you only have to make up two thirds of what's left to run even. If they do 100 damage per AA (for the sake of simple math) they will take 30 damage in return. Let's say you reduce it by 50 with your armor. They still take the 30 damage but you only have to do 21 damage per their AA to make progress. If you have thornmail it's easy to assume that you have more health because you are most likely a tank therefore making this an advantageous trade. Personally I like to combo it with Sunfire Cape. Thornmail is usually not an every game item (unless you have a taunt) but it's one of the best situational tank items. It's just a matter of how you use it. I hope this was at least a little informative, and will help you in making the decision of whether or not to buy it each game.
