I had this happend to me about 10 days ago.
Its currently a bug on the live servers. It happens when you are at 4 stacks of ferocity and you use your (E) Bola to gain the 5th (Empowered) Ferocity stack. Ever since 6.2 he has had a lot of bugs but on 6.9 the next patch most of Rengars bugs will be fixed according to the PBE. Hopefully this helped.
