Rakan's second E cast
It won't work if you don't have enough mana. It says on the tooltip the second cast is free, but if you don't have mana then it will not cast.
To recreate just play rakan, use e, run out of mana, it won't let you e again.

It's a bigger problem! Your E costs 4 times as much mana as it should. For example, if it costs 90 mana according to the tool tip, the first cast costs 180 mana and the second cast costs 180 mana so it's a huge problem.
First cast takes 200% of the manacost, second "free" cast takes an additional 100%. So Rakans E currently costs 300% mana, BUT only if used on Xayah.
Dang, riot had it out for him so hard that they had even bigger nerfs planned xd
