Do her passive bonuses stack? So if she has 500 stacks, does she get the 100 stack bonus, the 250 stack bonus and the 500 stack bonus, or just the 500 stack bonus?

100 Stacks: 5% increased ability power, and Twin Fang now heals for 6 / 8 / 10 / 12 / 14 / 16 (+ 10% AP). 250 Stacks: 10% increased ability power and 25% bonus cooldown reduction. 500 Stacks: 30% increased ability power.
So I think if she gets 500 stacks... the AP is only in 500 stacks... but the CDR still gain from 250 stacks one... and the healing effects from 100 stacks too...
Yes the effects accumulate. You don't lose any by reaching the next tier.
