does this mean .27 of your max HP or literally .27 HP every 5 seconds?

.27hp5 27% per 5 seconds would be crazy XD
If you get 9 of them, then you should have 2.43hp5 more at level 1. For comparison, at level 1, Soraka and Gnar has 2.5 hp5. (lowest hp5), and Heimerdinger has 11 hp5 (highest hp5).
