More importantly, what in the world of Runeterra is strong enough to permanently scar the most powerful Void creature (arguably), one with INCREDIBLE REGENERATIVE POWERS?

Taliyah has a quote against Baron :"As before, we will stand against you.", maybe Baron used to terrorize the people of Shurima thousands of years ago and they had to unite against him to get him away of their territory. He has to have popped from the Void somewhere and usually it's Icathia/Shurima.
Probably one of those time Dignitos almost got Baron but got aced instead.

Remember this powerful Voidbeing from Aurelion Sol's story? Since the big bad Baron Nashor is a voidborn without the ability to speak, so a beast, it might've thought it could attack the Lord of the Void, following instincts. Thus the Lord slapped Baron really hard and exiled it to the mortal plane, Runeterra. This is just a theory.
Welp, after you beat it over and over again and it keeps spawning ofc it gets a scar sooner or later :P
Jokes aside, i think the scar is just that - a gameplay-esque symbol to signify that it gets defeated over and over again without being really killed.
- Olaf. In his lore he tried to die in battle only to survive multiple times, could've fought nashor.
- Whoever got the first Nashor's Tooth.
