how does sona communicate within the various regions of runeterra?
does runeterra have a universal sign language? does she always carrying something to write on? i doubt she communicates telepathically outside of those shes very close to often.

Right now, this is still unclear.
There is certainly no commonly accepted language across ALL regions of Runeterra, let alone sign language. We default to writing everything in English, even though there's probably a remarkably varied spread of languages being spoken across various stories and such. It depends on the POV of the narration, really.
A TV show which handles this really well is 'Vikings'. Even though the whole production defaults to modern English (with some accents!) you can assume when the Viking characters are speaking to each other, it's meant to be in an old Nordic language. When English characters are speaking to each other, it's in one of the old English dialects... and the French guys, and so on. BUUUUUT when they try to speak to each other, the language barrier slams down HARD, and the scene is presented from the POV of whichever characters we're following at the time - some of them speak several languages, which means they can move in and out of conversations.
If we're making Runeterra a more authentic and realistic place to tell stories and play games, a major thing to consider is how the people in that world communicate. I am from the UK, and I can speak a little bit of French and German... because geographically they are pretty close by. But I can't speak Italian, Spanish, Hungarian, Polish, etc etc. I am utterly clueless with Korean, Mandarin, Japanese, etc etc.
Soooooo... can someone from Ionia like Sona understand whatever language they use in Demacia? Probably, because she lives there. But can she understand someone from Shurima, or Targon, or Ixtal, or even the Freljord? That's unclear.
Also, can someone from modern Shurima understand the ancient languages of their homeland? All good fodder for future storytelling! :-)
She can convey emotions with her etwahl. However, we don't know what she is doing currently. If the Ryze cinematic is accurate she is living near or in a monastery in Ionia. I can imagine her powers being sufficient to communicate in such a spiritual place. (Also, she has no telepathy. That is purely a game thing.)
Even Riot doesn't know. Some old champions have some very big plot holes and Sona is one of them
