He continually rewinds to a certain few seconds/minutes before hand. When Ekko first found his friend dead, he immediately backs up time, and constantly runs through every possibilty he can to save his friend, untill he reaches an optimal time to catch his friends last moments.
He's then given a choice to either take revenge on the man who shot him, or to be with his friend before he dies.
Also, i believe that Ekko's device works by setting a moment in time, then reverting to that point when he rewinds. Using that theory, he most likely had set his point in time after hearing the gunshot that set his friend to death, so not matter how many times Ekko rewinds, he will never be there in time to save him.
The worst part of all, he has to experience his friend die 92 times, and he might've gone back more if hadn't been forced to choose. Imagine what that would do to a kid.
Because his friend didn't die when Urf was up.
The device doesn't seem to follow the rules it does in game, which is 4 seconds back. Unless he was able to traverse the whole city in 4 seconds.

If you have seen anything that involves any mention of butterfly effect in it you'd understand.
It resets to a particular time; imagine that he 'saved' at a particular point.
He couldn't go back any further in time because his Zero Drive is either broken or not complete, I can't remember which, and so he go only go back in time a little.
