Blake -- The Unseen Hunter

Lore: Born in Ionia, Blake always loved the wilderness. He loved hunting and just the overall freedom of being in the woods. He spent most of his time hunting and gathering for his family. After years of hunting in the Ionian forests, learning new tricks. Blake became one of the best hunters around. Mastering the bow and knife, learning trap techniques and finding new ways to capture his next meal. During the Noxian invasion, Blakes village was destroyed. He vowed to avenge the deaths of his family and friends. He spent the next years wandering the wilderness, killing any Noxian he came across. Like a ghost in the night, shrouded in legend. Many speak of this hunter in the woods. But no Noxian has ever crossed him and lived to tell the tale. And no other have ever seen him.

Abilities:

Passive: Ghost in the Woods -- While out of sight of enemy champions, Blake gains 10/15/20% bonus movement speed and 5/10/15% bonus attack speed. After being seen by an enemy champion, Blake loses the movement/attack speed and his next basic attack will have a different effect depending on which weapon his has equipped.

Knife: Blake's next attack on a champion deals 40/70/100 bonus physical damage and slows then target by 45% for 1.5 seconds.

Bow: Blake's next attack on a champion deals 30/60/90 bonus physical damage and causes the target to bleed for 10% of their max hp over 4 seconds.

Q: Evade/Tactical Takedown -- (No Cost)

Evade: Blake will do an evasive roll in the direction of the cursor (300 range). (Must have bow equipped)

Tactical Takedown: Blake lunges at his target, dealing 60/75/90/105/120(+75%AD) Physical damage and pushing them to the ground (roots them but theres an animation of them on the ground).

Cooldowns: Evade -- 12 / 10.5 / 9 / 7.5 / 6 seconds.

Tactical Takedown -- 15 / 14 / 13 / 12 / 11 seconds.

W: Master Hunter -- Blake switches weapons. (3 second cooldown) (No Cost)

Knife: Blake takes out his hunting knife, losing 500 attack range, gaining 10/15/20/25/30 attack damage. And changing his abilities.

Bow: Blake takes his bow off his back, gaining 500 attack range, and 10/12/14/16/18% attack speed. And changing his abilities.

E: Tripwire/Strike a Vein -- (No Cost)

Tripwire -- Blake sets up an invisible tripwire on the ground, (200 units long) when an enemy walks through the tripwire, Blake gains true sight of the target for 3 seconds and they take 40/50/60/70/80 (+10%AP) magic damage and are slowed by 60% for 1 second.

Strike a Vein -- Blake stabs at a vein on his target, dealing 50/65/80/95/110 (+80%AD) Physical Damage. If the attack hits a vein (veins are marked similarly to fioras passive) it will also cause the target to bleed for 30/40/50/60/70 (+90%AD) physical damage. If Strike a Vein is cast during Tactical Takedown, blake will deal 25%bonus physical damage.

Cooldowns: Tripwire -- 20/18/16/14/12 seconds

Strike a Vein -- 15/14/13/12/11 seconds

R: Rangers Onslaught -- 90/70/50 second cooldown) (No Cost)

Blake gets stealthed for the next 6/8/10 seconds and gaining 30/35/40% Bonus movement speed. Attacking briefly removes stealth (similar to Akali w, just without certain area) During the duration, every 3rd attack on an enemy deals 2% of their max hp as bonus physical damage.

(Must have the bow equipped to use this ability, cannot switch weapons while ability is active) (Blake can be seen by control wards, turrets and nearby champions)

I like how you didn't include an image. Keeping with the theme.

Passive: Mostly fine, but a 5% max HP bleed and a 40% slow are not even slightly comparable. The slow is just so much better, even before accounting the range advantage. Try reversing the effects and bumping up the bleed in exchange for lower flat damage.

Q: Alright, but low on damage there, but I get it. Just for clarity's sake, here is the proper cooldown chart, since it goes by level. (12/10.5/9/7.5/6) (15/13.75/12.5/11.25/10)

W: The movement speed bonus is ridiculous. No, just no. Having permanent % movement speed like that is just... Dumb. Also, it only changes his Q, since his passive is accommodating. Not really needed to specify that it changes his abilities when his Q accounts for it.

E: Duration of true sight? Size of tripwire? Method of placement?

R: That is utterly broken. You'll note that the longest non-conditional invisibility in the game is Shaco Q, and he is both melee and an assassin. Other people with invis are in roughly the same boat. Imagine giving Twitch back his invisibility and then letting him turn it on or off at will for the duration. That's basically what this is.

I keep imagining a cartoonish white guy with frosted tips in generic stereotypical safari clothing. Maybe it's just because his name is....Blake.

In terms of feedback, it seems like you will rarely be in knife form except for 2 seconds at a time to Q someone who gets close. No real point of the dual forms imo.
