Players who refuse to play their lane

Can anyone from Riot (or with links to a Riot post) please provide some information on how this should be reported to best bring it to Riot's attention? Obviously not for one game, but to ensure that the report, along with others (if they happen), is noticed? Games where the support is top-lane more than they are bot-lane are not fun for the ADC, nor anyone except maybe the person trolling by doing that.

I usually just report for negative attitude with a very detailed comment, and then check verbal abuse and intentional feeding if they also apply. Is this the best way to do it? Does it qualify as intentional feeding even if they never ran into fights, etc (as they are leaving their adc in a 1v2 lane the entire game with no help)?

Refuse to play from the start file a report for sure...

But what I would like Riot to address is how you are playing any solo role Top/Mid, and your jungler keeps coming in and intentionally throwing the entire lane phase causing the enemy to snow ball, even after you said to stop.

That is the best way to do it, but no one said that support has to stay with the ADC.

As long as you hit the report option that is enough to flag the game for review be sure to include the reason to the report incase a rioter is manually reviewing the case if you want more clarification riot support is probably able to help

Reporting someone automatically flags the game. But is it punishable? It all depends. If you continually do it, and it's obviously unviable, and it's obviously ruining the game for your teammates, YES. Riot has stated this before. Going off-meta is not, unto itself, punished, but if you're not cooperating or communicating, and screwing over your team, and doing so repeatedly despite the obvious fact that it's not working, it can be punished.

Nobody is actually going to be punished for that unless they are doing it very often. If someone is doing that only when they are autofilled support, then they will never actually be punished. You can report them in game for it, but that won't do anything in regards to them not playing their role.
