Does that mean the person I just reported got their final offense tallied against them?

Does that mean someone I reported months ago got action taken against them?

Does that mean they had enough people from the game I just played with them reporting them that they got an immediate notice?

I'm just curious what the effect of this is, practically.

It means that a player you recently reported has been punished by the automated system, and that the match you reported them in triggered it. Multiple reports don't "stack", per se, because punishment is calculated per match not per report, so they would've likely been punished regardless of how many people reported them in that match ("likely" because you can lose weight in your reports for making false ones).

The Instant Feedback system works within 15 minutes, so it'll always be a recent match that triggers it. Though, if you join a match before they get punished you might get the message after you come out of the next one.

That is a rare occurrence, that usually means that your recent report (may take a few minutes after issuing the report) caused them to have action taken against their unwanted behavior. What kind of action? That info is not shared nor needed.
