This is a serious question, I've heard people who were never warned, never chat restricted, and probably wasn't toxic in general get banned, just because they had a really bad game and flamed. I don't know Riot's stance since there's really no way to see how close a player is to getting in trouble, which I find quite dumb. I don't know if it's happened to anyone before.
It depends on how serious it is. Usually using racist, homophobic and other derogatory slurs can net you an permanent right off the bat. I mean do you really need a warning to tell you that its not ok to say things like that?

The answer to the question in the subject is "Yes you can be banned, if you flame badly enough." You can get permanently banned for the first ever offense if the flaming can be considered hate speech/racial slurs/death threat etc. I recommend "mute" instead - and not listening to all chat in the first place.

I remember when LYTE would SMITE all those would-be one ban wonders. More often than not people are lying when they say they got banned for one bad game

Just think of it like this man. If you wouldn't say it to someones face, don't say it online. I know some folks think they are crazy tough but I have never in my life, even in sports, heard players talk to each other like that. There was a pitcher in baseball that went off once, he's not a pro pitcher anymore. Just don't flame. It isn't going to help and some folks really don't like people that do flame in the community. Remember most folks are banned by the tribunal. That's your community deciding they don't want you in it anymore. Just don't be a dick.
I got a 14 days supension that ended 2 mins ago (yay!) for saying kys to a friend in the team chat. I'm not toxic at all, I barely talk unless if I play with friends (we don't discord/skype)
