I feel like a lot of behaviors could be avoided if people were informed ahead of time that they are reportable behaviors. It probably says it in the Summoners Code, but I think visibility is a problem when it comes to things like that.

It's sorta like Municipal codes. They are posted and you are expected to know them.
It's probably a priority below improving the new player experience... So not likely.
keep in mind that the players are the ones deciding the rules as it is an evolving system based on what is being reported.
If suddenly people feel that asking for reports is not harassing / negative anymore (stops reporting on that issue), the system will learn from that and be less punishing to it.
Riot will never be able to keep up with the world of toxicity if they were the ones making the rules.
