Just had a 4 man premade with the most Toxic Sona I have ever met. Non-stop trashtalking me because we lost. They ignored me when I needed help and then I get flamed. I had her in my next game and reported her again. Riot, if your system doesn't punish this person, I will lose all my faith in you. A player flames me 2 games in a row, and I haven't reported anyone else, so since she's my last report I should get the notification.

Not how the system works. If your reports triggers the review is when you get a notification. If she gets triggered by another report in a game after, you won't get a notification.

If you report someone and they get reviewed after that game you'll get notified. If they play one more game and someone else reports them you won't get notified. Only the people that report him in the last match and trigger the review get a notification.

For example, it takes 10 validly reported games to get someone punished, if you've reported them on the 9th game, then you won't get the notifiaction. If you reported their 10th game, and they got punished that game, you'll get the notifaction.
of course if you reported them only on their 2nd & 3rd game, you won't get that notification at all.
But each of your reports are independent of each other, thus the more you validly report, the higher the chance you'll see the notification.

Void Nargacuga1 votesNot necessarily. The review of any report always happens. That message only really pops up if your report which triggered a review, which lead to a flagged match, ended up causing a punishment to be doled out. (I'm sorry if that sounds confusing)
All reports are checked for validity. Always. The clarity of the instant feedback system could use some work. I've heard that this is being worked on and could potentially arrive with the new client.
