If someone is cursing their head off, and I forget to report them. Does the system simply not punish them? Or if someone AFK's and they have the redded out name, but I forget to report them? OR if someone has an vulgar or racist name, and I forget to report them?
You get the idea.

Reports are significant on a per-game basis because no one is punished on game data alone. So if there are no reports in a given game, the system assumes no one had a bad time, and there’s no reason to consider punishing a player. This means that if you play with a negative player and don’t report them, they won’t receive a punishment and likely won’t reform.

They could still tune it as they imply they do, but if I forget to report someone because I was talking with friends, or simply eager to play another game. Then this individual should still get punished to prevent them from ruining the games of others. It seems as if it's a system with little to no room for simple error. This is why there is so little absolution to the toxicity of the player base tbh.

If you forget to report a player, you can submit a ticket to Riot Games Player Support, who can review your report further. It won't be included in the Instant Feedback system, but if it's consistent enough or extreme enough, Riot can take action on that player. Again, Instant Feedback is in place to identify players who are creating a negative environment for other players, and if neither of the other 8 players in the game report a player, it should be pretty safe to say that the player didn't do that (ie, statistically unlikely). However, manual reports are there when all else fails.
