It is Riots fault. They do absolutely nothing significant to rid the game of the people you describe. We should not have to be tolerant of assholes and seeing as Riot has decided to continue down their path of acceptance of these type of people, they will eventually have no game to fix.

Not quite. It's not so easy to just get rid of these kinds of people. There are so many factors to take into account. How severe behavior was, how many times this offense has occurred, in what scenario did the offense take place?
Also many "trolling" cases cannot be persecuted. You can't penalize ADC soraka stacking MR against an AD team. There's nothing in the rules and justly so that you are not allowed to utilize the resources given by Riot, such that you are restricted from purchasing certain things just because a meta or situation might call for a better option.
You can't persecute a constant split pusher who is fucking your game over and not team fighting no matter how much you ask for him because he is still completely in the borderlines of the rules, and is still assisting the team by applying pressure on objectives.
You can't persecute someone for being "bad". If your Elise went 0-12-0, that's not reportable. Unless she fed intentionally, that of which can be deduced from replays, there's no persecution for that.

Feeders should be persecuted as long as it is intentional. We've all had games where some guy gets in an argument and just runs down lanes dying. Replays will make that evident, especially since said player typically announces they will now feed. But, again, can't persecute someone for dying a lot unintentionally.
Trolls: ADC Soraka, jungle veigar, Tank Zed, triple mid. None of it is reportable. You simply can't force players into building what the meta dictates or go where the meta dictates.
Toxicity. This, in my opinion, is the most difficult. What is toxicity? There is a massive difference between saying "Wtf ur stupid" and "Uninstall league you cancerous little trash shit". There is something to establish here. This isnt Club Penguin. Some level of toxicity is normal, and Riot has admitted that they believe a bit of trash talk can be healthy for the game, i.e competitiveness and improved performances. If someone calls you bad, that really isn't an offense. Even going so far as to be called retarded or stupid, or an idiot isn't really that bad. Retarded maybe just because of the implications of the word. It's when people are told to catch cancer, called a %%%got, are dehumanized is when toxicity is critical and riot does take action against that. So when you're called a dumbass in game for not backing or not warding, don't try and report that. That's fine. This is a game not meant for kids so that's normal. It's when they start attacking you personally that you can report them. And yet again, you never know the history of that person. They could have never, ever been toxic or gotten reported and yours would be the first real one so Riot could decide not to take immediate action but sort of put that account on their own little probation list and if it continues, then action will be taken. But it's also unfair to just slap on bans and restrictions off of a single offense.
