It's something that's already been brought up around Riot. I'll probably look for a follow up later on.
IMO for summoner name just use the in-client feature, for club name i'd send in a ticket because i'm not sure club names are included in the actual ticket submissions as a variable that gets passed along in the report.
Support, there should be an additional report system for different types of reports. This should include Clubs, because Clubs require care too since it is a part of the game.
