They can and do! Most of them keep their names, but I've seen a few change them, such as Gypsylord and Shogun.

Once I did a summoner name change without swapping around which account was flagged as my "riot" account. Got a lot of "Is Gradius no longer a Rioter?" questions soon after that :S

So yeah, we're able to change our summoner names and sometimes it causes confusion if we don't plan it correctly.
