it is usually the champions they last used to win the tournament, but there are some cases where it is the person on the team "best known for" champions.
Duke: SKT Ekko (Confirmed) Blank: SKT Zac (Confirmed) Faker: TBA Bengi: SKT Olaf (In Consideration) Bang: SKT Jhin (Confirmed) Wolf: SKT Nami (Confirmed)
Not sure on the release dates, also the source i got this from says these might not make it with Riot but that it's what they'll choose

this was posted shortly after worlds, i know someone said they usually make skis for the champs used to win the final match but i've also heard the victors get to choose their desired champion to get a skin
"Edit: Here's the source - http://sports.media.daum.net/sports/esports/newsview?newsId=20161031104524183 Since it is in Korean, I'll translate an excerpt from it "During the discussion with Coaching Staff, the members of SKT have mentioned the champions that they want skins for. Faker wants either SKT Ahri or Syndra, Duke wants Gnar or Ekko, 2 Junglers have not yet decided, Bang has chosen Jhin, and Wolf has chosen Nami," (Daily eSports)"
Yes, the players choose a champ and Riot prefers the champ that they used most at Worlds (from what I understand). Riot also works with them (like last year when Sivir was taken out and art was not good enough) to make the skins better. Though, this year they will work more closely with the players.
Since Jhin just received a Blood Moon skin, I doubt he's getting a SKT skin on top of it. We'll see what Riot officially posts.

Riot doesn't count team skins as skins for champions in their rotations of who gets a skin next. In other words if Cass got an SKT skin, she would still be in line for a new skin because Riot doesn't count team skins as real skins. So even if the chosen champs did get a skin, Riot would still make them another one for the team
It doesn't really matter if Jhin already got a skin, E-sports skins aren't looked at as part of the champions skin catalog. So they can receive them even if they have another skin come out shortly before then or vise versa.
It's like how both of Kalista's skins are infact E-sports skins and she has yet to get one that isn't.
