I'm Diamond. It took a lot of focus on learning matchups, map awareness, mastering mechanics, and knowing when to make plays but I have finally reached Diamond as a support main! I started silver when I first played, by the end of the next season I was in gold, a year after that and plat, and now this year I have joined Galio's Beserkers in Diamond rank.

I just takes practice and lots of it, also a bunch of studying when you first start because there are so many champions and ways to build them. I've found map awareness and positional skills to be paramount for climbing up the ranks after I reached Plat where as mechanical skills and ability to make well timed plays worked wonders in gold.

StoryDuck1 votesSure, I see you play a lot of Chogath. I see you build the same items regardless of matchup. Protobelt is strong yes but you don't always want it as a first item. The dash is great for Cho but sometimes you will find you need to play defensively and scale up a bit before you start 1 shoting fools. Don't be afraid to buy a defensive item as your first item. If you are against AD a quick 
can work wonders for your survivablity. Against an oppressive AP champion try getting 
first instead. Every game you need to analyze what you need for the situation at hand. Guides online are great starting points but they can't cover everything as there are just too many different matchups. If you don't think you can win lane endure it with some more defensive itemization. There is of course other things to work on, map awareness, champion mechanics, positional skills, wave management and you might want to brush up on refining your CS skills as your cs seems low for many of your games.

StoryDuck1 votesAh, in that case you need to work on wave management. Meaning, finding ways to setup the minion wave to allow you to roam without losing much CS. Things like killing 1-3 casters in the back to setup slow push, the +4 minion freeze and pushing waves with the correct timing. This video is pretty good at covering the basics:
