Discuss: Some of your biggest screw-ups, weirdest or most unusual confrontations, games where you felt completely overwhelmed, etc.
I'll start:
I just came from a TB game where I went Ezreal ADC. The entire enemy team (aside from one gold player) was platinum. I'm just an unranked scrub and ADC is one of my worst roles. 
One time in draft mode, it was my job to ban. I forgot we were in draft mode, and immediately selected Lux as soon as we got into the lobby. You know, the champion I really wanted to play.

I was playing Caitlyn and running from a gank at bot lane. Their Thresh was the closest to me as I tried to E to my tower. Of course I spasm my hand in panic and end up E'ing in the complete opposite direction, towards river. The funny thing was, as I did that Thresh threw out his hook and it almost looked like I predicted and juked the hook. I still died of course.
Started a game, playing as a mid-laner, and sent my champ (might have been 
but I wont swear to that, as it was a while ago) to start auto-walking into lane while I was looking at the team screen and the shop. Apparently I clicked to far on the mini-map by mistake and while I wasn't paying attention to the main screen, I walked right into turret range and was executed right then and there. Woops!
Yeah, that was pretty embarrassing/awkward. :)

I was 
and had 50-70 hp. Than a 
jumped to me with 150-200 hp. I did Rappel when he comes, I quickly did Q to my surprised enemy and killed him. I went to my base without dying. What an awkward moment :D
Last night i go top lane see that the gp started 
laugh a but then proceed to get crit on his first 5 autos in arow and die 5 in a row 8% crit chance each .... check his runes no crit chance .... 5 times i was traumatized 5 times in a Row O_o
That awkward moment when one game your ping is fine and the next it's constantly over 1000 and you somehow manage to get 3 kills anyway.
I played a game a few days ago, where 2 players on the enemy team went afk- the support d/c'd early on, and their gp top left the game around level 10. He came back RIGHT after the game ended in the post-game lobby and asked "Did we win?"
I don't believe I ever facepalmed so hard in my life.
A while back I was rolling in Normals alone and I got into champ select and we all called jungle. We then decided fuck it 5 man jungle. We dubbed ourselves the
JUNGLE FORCE FIVE
and then raped the enemy team for the next 25 mins including a level 2 dragon and a baron the second it spawned. Unfortunately they had an 
free farming top lane and we lost.
It was wurf.
I once was going to play Janna support, called support, selected Janna, waited a bit, 10 second left, I locked in. Then someone randomly locked in another support with just a second left on the clock. I wound up going top. I forget who I was matched against but the game did not start well, I died once really early. Then 3 people tower-dove me at about 10 minutes, I got a triple-kill somehow, and I wound up carrying the game. I generally don't worry too much about seeing unconventional picks anymore. It was a very stressful and awkward game but in the end it was very fun.
In ranked, a teammate and I agreed to pick up each other's adc and support (leona and jinx), so I picked up the guy's leona for him. Only one the match started did we both realize that we'd forgotten to swap... but we still both did great and ended up winning the match lol. xD
I was playing singed in normals. We had no vision on the enemy team so I just put on the ulti and ran into there base.... Seems bad but it ended good I ended up baiting there entire team into a bush full of teemo shrooms(teemo on our team). The team instantly blew up and teemo and I killed everyone... Not an awkward moment for me but it sure was for them

If I may contribute, I was in an ARAM as 
, rocking my Zhonya's like you do. The enemy Gragas and I had a pretty adversarial relationship. I wasn't playing my best, and I chased Gragas down the lane. I popped my ult too early and my E was on CD, so I wasted my entire ult. Gragas hops into all chat and says, "NICE ULT." -.-
Not twenty seconds later he chases me into base. Both our turrets were up, but being Gragas he dives me anyway, thinking he can drop me quick and get out. I Zhonya's perfectly while he's in midair. It's got to be the fastest I've ever gone from an awful scrub mistake to a pro play. He runs away taking a beating from both turrets, and then I come out of stasis and shuriken snipe him for the kill. He hops into all chat again and says "NICE BAIT." ^_^
After I first started playing league a buddy of mine invited me into a game. I was level 10. He was diamond rank. Needless to say I got smashed.
I was playing Normals one day, and almost everyone lagged out, about the 7 minute mark, So I ended up On defense 1v3 as the awkward moment was when I held the base by myself for 13 minutes till I could surrender.
That moment where you're Caitlyn and fire your net to get away, but the game lags at that exact moment and propels you into the middle of the enemy team. Or you're cornered in the back of dragon/Baron and you try to jump at the 1 spot on the whole wall that's impassible because the graphics don't display it.
A bit over a year ago I solo queue-d for a game and apparently one of my friends did at the same time. We ended up in the same champ select and were all excited and then someone dodged and he ended up on the other team >.< He carried his team as mid Kennen and kept trying to make excuses for focusing me, the support Sona, all game.
I was doing a Draft Pick. Picked Jinx and had a Morgana support, thought we were good. Next pick by the enemy team is Sivir and with a Karma support.
First thought, was I can still get this. Sivir isn't that hard of a counter... the Sivir had the Season 2 Summoner Icon, Karma had the Traditional skin.
Worst game of my life. It physically hurt me to play that game.
I went into a game that was today actually in team builder with friends, i ended up disconnecting and reconnecting over and over again due to ping. I went 9/3/5 as
I was playing a ranked game as Draven when my brother accidentally hit the power chord to my second monitor causing it to shut off and reformat to one screen. It was during a big team fight and I thought i would die instantly. Turns out when i got back into the game I got a double kill and we pushed to win while i stood in the middle of mid lane trying to get my monitor plugged in.
My friend invited me to play a match with him, and I picked
top. Well our mid- laner never connected so he (as jungler) took top and I went mid... Against 
... I got stomped...
Back when I first started playing, I would play Morgana with a mix of weak AD and Armor items.
... I didn't know how item builds worked back then...
As I was walking into a tri-bush low health as a support, the enemy team support, who is also low, walked in at the same time I did and we both went to base at almost the exactly the same. Surprisingly we both did not kill each other and we went to our respective base. Most awkward 5 seconds of the game for me.
After ganking mid lane and successfully getting a kill as Nocturne, the enemy Blitzcrank hooked and killed me as I walked out of the lane brush and into the river.
