lol Where did you get those numbers?

150 / 250 / 350 (+ 55% AP) is the base heal for Soraka

250 / 400 / 550 (+ 60% AP) is the base dmg for Karthus

BUT if Soraka's allies are below 40% HP her heal is increased by 50% to make up 225 / 375 / 525 (+ 82.5% AP)

Typically Karthus will only ult after they are really low so her 50% increase will almost always be a factor against Karthus, the R from Soraka would heal almost all of the damage it would deal. The only way it would kill them is if they are EXTREMELY low, not have great resistances, or Karthus is extremely far ahead of everyone else. There are a lot of situations where Soraka would save someone, it's why Soraka counters Karthus often

Anyway that doesn't make the scaling go up it just increases the heal. So if she had 100 AP with a 40% modifier from items her ult would go from 205 to 287 at level 6. I don't know if it makes a difference in the amount healed though

that's completely worthless on his R. on his Q and E it's great, but his R is pointless it doesnt give you any grievous wounds effect against soraka. the heal comes first.
