Ravenous hydra is your core item, i prefer building it first every game.

Your second item depends on their comp, either Trinity force against squishies or Black cleaver against tanks.

Third item should be a somewhat defensive item again depending on the enemy comp. If you're snowballing at this point consider building death's dance or guardian angel 3rd. Guardian angel is brutal when bought early on super fed champions. Enemy team usually has no idea how to deal with it. Maw of malmortius is also a good choice if the enemy team has heavy magic burst for example champions like Annie or Syndra.

After those 3 items I prefer using my last 2 slots for defensive items, again depending on the enemy team. Consider things like are there any heavy auto attackers or champions that rely on crits. Yasuo is the most clear example. I look for building randuin's omen every game i'm against yasuo. Especially if their adc is someone like tristana who relies heavily on auto attacks.

Spirit visage is a good choice against consistent magic damage and poke, you should be looking to buy it against champions like teemo, swain, xerath, orianna, vel'koz and ahri.

Other options are items like sterak's gage if maw of malmortius isn't enough to deal with the heavy burst or if there is a troublesome ad assassin on the enemy team, and frozen mallet if you're so ahead that you don't need to build a tank item. Mercurial scimitar is also a good idea if you're against reliable cc, for example malzahar's ultimate.

Your boots should be either ninja tabis or mercury treads depending on if the enemy team has a lot of cc to lock you down with or if they have a lot of basic attackers like master yi or draven.

Also there are some exceptions in your early build path. For example against mundo you want to always build executioner's calling early.
