There is an item called The Mundofucker and it looks like this and the healing prevented by it is much much more than 15% of your physical damage, no matter how fed you are or who you're playing as. Remember, there are only two sources of grievous wounds from items, and only applies it when the target is below half health. These are the only items that can keep Mundo from ulting just before he drops below 50% and avoiding GW altogether.
