To Self Cast Lissandra's ult, I currently have to press ALT+R
Is there a way to change the key bindings from ALT to another key? I'd love to change it to something a little more comfortable, like Tab+ R or Spacebar+R

Once in game you should be able to adjust key bindings by pressing esc. I'd suggest going into a custom game so you aren't afk for five minutes trying to find it.
You can go into the ingame options, under keybindings, there is a section labeled "Self-Cast" with spells 1-4. Your ult is spell 4, and you can set your ult to either R for self cast or S for selft cast and still have R as normal cast.

The answer my friend is a gaming mouse. I recommend a razor mamba - it's the best mouse I've ever used. You can bind a thumb mouse button to alt (I also bind to H for standing still and auto attack kiting.) It takes some getting used to but after a few games it's super natural.
There are gaming keyboards to that let you bind keys too BUT the mouse will work better.
