Remove Nocturne from Ascension Mode, or alter his Ult to fit it

It's one thing if you're getting overpowered by Xin Zhao or Jax, or an ascended Fizz; it's another when your team is prevented from targeting or even seeing Xerath and then seeing that your opponents got ascended instead. Because now those last-millisecond clutch moves with your AAs and target abilities now go down the drain, and pretty much nothing but a lucky LeBlanc blink will counter it. Or perhaps they could remove it so that you can still see Xerath, but your sight of everything else will still be removed. It should make sense, considering you're always revealed in Ascended Form, whether by either Passives or abilities.

Isn't that the point of the nearsighted debuff? To remove vision during crucial moments?

Yeah, pretty much the EXACT point of nersight. Why don't we remove Graves, Tahm Kench, and Quinn while we're at it, they can actually do it more often.

Graves ultimately has to get close enough to get aggro'd, Tahm pretty much has to get aggro'd (and while I haven't played Tahm I heard there's a bug where you can't eat Xerath, Champ or Ascended; not sure if it's true or not), and Quinn can only affect 1 opponent with the nearsight. They might be able to use theirs more often, but Nocturne's is a global, affects-everything nearsight that nothing but QSS and Banshee's Veil can counter (and probably something else).
