@RiotRepertoir: What are the main factors that decide which Champ gets reworked?

I know it isn't simply an issue of play rate and win rate. Ashe and Trist were both doing fine before their respective reworks.

Is it more of a kit cohesiveness issue (see: Sion) or trying to make the kit a little more interactive (see: Darius)? All of the above? How do you guys identify what parts of the kit need to stick around and what gets the axe?

Basically, I'm just really curious about the whole design process.

No, no they were not. Ashe wasn't taken seriously at all and besides solo queue saw no play, her ulti is and was amazing, but her damage is lane was piss poor compared to what the other ADCs had and could do on top or her E being subpar. Tristana was in a better state, but she was also on the weak side, honestly, I felt like she needed to have some of her last nerfs reverted rather then a rework.

As for why, it could be a couple of reasons, with Sion's case, it straight lacked any cohesiveness, besides that one guy who said one side was better and acted like he was viable, Sion was a mess. Either you build AD and were useless early game with your stupid high mana cost, lack of ranged damage or damage mitigation, or you built AP and were useless end game with your half of damage also being you ability to survive fights, it didn't meld properly and half your kit was useless depending on which path you went and if you hybrid, congrats, you were a shitty combination of Irelia and Aatrox.

In Darius's case, interactive but also he had fallen off with all the nerfs he had been getting to that point. He was a counterpick, but couldn't really carry as a teamfighter unless insanely far ahead.
