Seems like in top lane where tanks and bruisers rule its better to build a tank item first like 
I always try to build items like 
but end up getting out tanked AND out damaged. Are defensive items just way better first item choices or what am I doing wrong?

At low levels of play cookie cutter builds are the best. Pretentious lowbies will talk shit about them, but until you've got the experience and knowledge to understand the items power spikes, costs, and synergy with ur champs you shouldn't ever be altering your build. So just go to u.gg or probuilds and find a good one and copy it.
In the toplane defense outscales offense if you are even in gold/xp.
Bruisers have high base AD so the abilities they have will naturally do more damage meaning they can buy defense and still get enough offense to survive for a long time in lane to kill you.
Also who do you play because matchups are 90% of toplane right now
