Now that Cleaver builds from Phage
only has 1 build option 
since Cleaver is updated to build from Phage
We also have a distinct lack of ways for AD champs to cut healing, especially in the top lane where champs like Mundo go and laners are almost forced to go teleport over ignite. Meaning certain champs just get to free farm and you have no kill pressure without assistance from other lanes or being an AP champ.
So what if they made a new item build from that could apply grievous wounds like 
The item could be something like:
Executioner's Calling (NEW)
Recipe: + 
+ 738 gold = 2950 gold
+60 AD
+10% cdr
+10 A.Pen
Unique Active: For the next 6 seconds, Dealing Physical damage to an enemy champion Below 50% health inflicts Grievous Wounds for 4 seconds. (60 second CD)
Now yes, this would be strong on some champions like 
(it is all stats he loves + the active removes a part of the counterplay to his ult) which is why it is just an example rather than what I'd actually expect to get put in game. And making it an active prevents it just being an auto win trades vs healing champs because you picked it up and can auto attack them.
Just an avenue I hope Riot could look at. And curious as to what others think as well ^_^
