The Surrender System and the inconsistency of anonymity.
To get right into it, I don't particularly see the reason why the identity of all but one vote while surrendering is kept completely obscured. The person who starts the surrender vote is listed in the chat and their vote automatically counts as a "Yes". Every other person on the team is allowed to vote anonymously. This is genuinely a little confounding, as it leads to a weird middle ground that I don't think especially serves anyone. It puts one person in the spotlight into what is, most likely, a discussion that should involve all five players on the team. I know this is anecdotal, but I have genuinely had games where people have just straight up lied about what they voted, with only the person who initiated the surrender vote able to verify their position. Why give people the ability to do that, when you could add the transparency and at least make it an honest argument if nothing else?
I feel like this is a genuine problem regardless of where you stand on the issue of surrendering, as it doesn't necessarily make the process of a surrender vote "easier" or "harder", but does make it more open and transparent rather than clouded with an unnecessary air of mystery. Depending on your current level of cynicism in your fellow man, it could lead to genuine discussion as people defend their positions, or it could simply oust the trolling duo botlane who are both 0/20
