The Issue with Cull

Listening to Pentakill's Cull and reading the comment section got me to thinking about the item a bit more. From what I gather, the issue with the item is that it's sustain is dogshit in comparison to dorans blade along with offering no hp, and one less ad. Of course these drawbacks are made up for with the free kill's worth of gold at 100 cs post purchase, but the problem is getting there. I was more or less wondering if it could be worth while to take on a marksman like Varus, Vayne, or Kalista who like to start cutlass first, which covers the lack of sustain from cull. Or even more generally, would it be worthwhile to pick up if the enemy jungle/bottom lane doesn't have too much poke/all in potential early? I'm silver, so I can't say that I am definitively good in many/any areas of play, but my farming is usually 180+/20 minutes, and in decent games, 310+ at 30 because I'm not the most aggro of players. I'm just looking for ways to increase income in the early/mid game and am considering Cull as a potential avenue. Any higher elo players can give their thoughts?

I think it's sustain is simply better than Doran's Blade for some champions with certain items. I personally prefer it over Doran's as Varus because when I get Guinsoo's Rageblade it applies the sustain effect twice since it's an on-hit based sustain effect.
