I've tested many things on him in custom games, played several games as him, he honestly doesn't seem as bad as people think. The issue is if he gets behind, as he is very item reliant. If he snowballs or farms a shit-storm, he can giga-rekt people with R.Hydra/Guinoos. I want to play him so much, but every time I pick, I feel like a troll pick, kinda like Morde (even though I have over 100k points on him).
He's too gimped to be honest. Building too much lifestead will lead to him being too squishy, thus being blown up before being able to lifesteal a meaningful amount. If he builds too much tank he won't do enough damage to be a bruiser and lacks the utility and disruption of a tank. If he builds both, he will fail at both.
If you want a champ similar to him, play Irelia. She still has the sustain, is fairly mobile, has good tankiness, and still does good damage, while also having decent CC.
Ive found atrox to be the perfect counter-the shit out of Quinn and vayne tops

what? Nobody thinks about ahtrox. He's considered the bad version of some other champ even though they are completely different, I forget which. This isn't exactly his meta though and sustain champs don't get a fair shake in any meta especially sustain metas.
He is probably intentionally niche but he's fine for the most part.
From my memories of playing him (really not a lot, just a couple of games) he seems like an unreliable Diana. If you don't get ahead in the eraly phases of the game you'll feel like a target dummy for most of the time. Once you finish 3 items though you come back into play and can perform well in teamfights.
But it's really nothing special and all you do feels unrewarding and clunky to play.
