Does anyone else think starting AD items could use a bit more distinction?

Cull is, of course, super situational. In nearly every situation Doran's is the better choice since it provides sustain and bulk.

But what if it didn't? What if Doran's lost the lifesteal and had its HP boosted instead? What if Cull dropped the gold generation, got one more AD, and some decent lifesteal instead of the OnHit heal?

So if you're against an all-in lane, you grab Doran's. If you're against a poke lane, you grab Cull. That seems like it would be a bit more interesting than 'always get Doran's every time'.
