Clarity needed

How exactly do the vitals swap places after you break them? Does it have a predictable pattern or is it fully randomized? At first it seemed to change to opposite sides after you hit them but now it seems quite random. Sometimes it changes to opposite sides but other times it goes to a completely random side.
