Do you ALWAYS get chest when teammate scores S?

I feel like I've played so many games where my teammate has scored an S but I didn't get a chest

It has to be in a premade group.

you can stock up to 4 chests, refill one every 7 days.

to get these chests, eith you or someone you are in a premade with will have to get an s score. additionaly, you can only get 1 chest per champ YOU specificaly play per season

so if you played velcoz mid and your friend played darius top, your friend got an s score you would get a chest. if the next game you took velcoz mid again but ur friend took gnar top and got an s with him then you, having already acquired your s with velcoz this season, you would not get one

is there a way i could know which champs i got a chest with already

in your profile page with the champion masteries there will be an indicator on which champ you've gotten your box with, there will be a special border around the champ picture on the old client and a box at the top of the pic on the new client

i'm probably wrong but i know it has a timer in which the amount of chests replenish over a week or a month, not sure
